Documents Needed for First Counseling Session

• 2007, 2006, & 2005 tax returns & W2's or 1099 if applicable


• Last 6 months' Bank Statements for all accounts


• Most recent pay stubs (at least 30 days)


• Name, address, & phone number of all employers for the last 2 years


• Name, address, & phone number for landlord(s) for the last 2 years


• Resident Status:
  - Resident alien card/birth certificate, voters registration, or US passport
  - Social security cards for all household members
  - Driver's license or State ID for all adults


• Report cards for all school-age children


• Immunization records for non school-age children


• $12.00 for an individual or $18.00 for a married couple, for credit report (check or money order only) This is non-refundable.
